Job Summary:

This role involves providing coordinated healthcare services for individuals in group homes.
Location:Â Bronx, NY
Hours: Day shift, Monday – Friday, Full-time
Pay Rate: Up to $46Â per hour + benefits (Health, dental, and vision insurance, life insurance, paid time off, referral program, flexible scheduling options.)
Responsibilities:
- Perform pre-admission and post-admission evaluations for new patients/residents.
- Conduct health assessments, recommend necessary interventions, and ensure accurate medical record-keeping in compliance with relevant regulations.
- Review and interpret medical reports, communicating key information to appropriate staff, family members, or healthcare providers.
- Collaborate with external healthcare providers such as physicians, dentists, and specialists to ensure optimal care.
- Oversee medication administration and related processes, ensuring compliance with regulatory standards.
- Valid RN license in the state of New York and an Associate Degree in Nursing.
- Knowledge of developmental disabilities and experience working with individuals with developmental disabilities is preferred.
- Experience in critical care, assisted living settings, or group homes is a plus.
