Job Description
HumanHire is seeking a dedicated and compassionate Care Manager to join our client's Home Care Department. As a vital member of the team, the Care Manager will primarily engage in telephonic interactions with clients, caregivers, and healthcare professionals.
*You must hold a NYS RN License to qualify for this role*

Responsibilities:
- Conduct remote assessments of patients' health conditions and rehabilitation needs.
- Collaborate with healthcare professionals to develop individualized care plans.
- Facilitate communication between patients, caregivers, and healthcare providers.
- Ensure timely and accurate documentation of patient data and updates.
- Offer guidance on exercises, medications, and lifestyle modifications.
- Serve as a liaison between patients and the rehabilitation center team.
- Advocate for patients' needs and collaborate with interdisciplinary teams.
Qualifications:
- NYS RN License
- Minimum 1 year of Care Management experience
- 2 years of client coordination experience
- Certified Case Manager is a plus!
- *Must be vaccinated against COVID-19*
